Summary of Position: The Clinical Liaison serves as an advocate for patients in the pre-admission process to Home Care and continuing until the patient is discharged from the facility to HomeCare Maryland. Liaisons work with patients, families as well as referral sources to coordinate transitions to home.

Essential Functions:

• Identify, develop, secure and grow new existing customer opportunities by building a strong network within the assigned area

• Perform pre-admission assessments of the clinical status of patient referrals and evaluate the patient needs in the home

• Actively assist the referring and accepting institutions in addressing the requirements of the specific patients

• Coordinates with the facility to ensure that each patient has a smooth transition through the discharge process and back to the home environment. Document all referral activity in HomeCare Maryland's electronic medical record system and E-Discharge/Referral Central

Qualifications/Requirements:

• Must have LPN/RN license with clinical experience dealing with patients and families

• 2-3 years nursing experience

• 1 year health care and medical sales experience, preferred

• Must demonstrate exceptional verbal and written communication skills

• Strong interpersonal skills

• Must have reliable insured transportation

• Home care experience preferred
